Dear Parents of a Hopewell Crest Preschooler,

  

         Welcome to a unique new school year at Hopewell Crest School! We are so pleased and excited to be a part of your child’s preschool experience. To keep all our students and staff as safe and healthy as possible, I am reaching out to you regarding our annual Flu shot requirement. Now more than ever our youngest Hopewell Crest School student body needs protection from the complications of Seasonal Flu. Your 3 or 4-year-old has the youngest immune system of our Hopewell Crest School family. Their body needs more time to build antibodies that protect them from getting the seasonal flu. The more protection your child has from the Seasonal Flu, the more protected all our students and staff will be from the Seasonal Flu.

 

            I am asking that you make every effort to get your Preschool child vaccinated with the Seasonal Flu vaccine as soon as possible in the month of September. The Seasonal Flu vaccine is a NJ Department of Health Preschool requirement. I would love to see all our Preschool students vaccinated for the Seasonal Flu before we resume in-person school days.

 

            I know this is a big ask but at Hopewell Crest School we Go Big! 😊 We ask more of our Students and Parents and Staff. If you have a child enrolled in Grade 5, the New Jersey Department of Education has developed the following Fact Sheet about Menigitis. All public schools are required to distribute this fact sheet to parents and guardians of every student entering grade 6. Upon turning 11 years old your child can be protected from Meningitis, Tetanus, and Whooping Cough by getting the Menactra (MCV4) Vaccine & Tdap Vaccine. Make sure Mrs. Barber gets a copy of your required Vaccinations. By School Law, students who are 11 years old may not attend GRADE 6 without the Tdap and Menactra vaccinations. Student not yet 11 years old are required to receieve these vaccines upon turning 11 years old.

New Jersey State Law requires the school nurse to check and record annually the height, weight, vision, and hearing of each student, Grades PreK-3. Starting in Grade 4 it is acceptable to screen students for vision bi-annually and hearing as needed and in grade 7. Scoliosis is checked and recorded bi-annually on each student after age 10 years old. Whenever a problem is found the school nurse will notify you by mail, and request a physician visit to examine the problem further.

How can parents help make participating in school sports a “no-hassle” experience?

  1. Keep a copy of your child’s current physical exam.
  2. Call your doctor to ask when your child is due for their annual physical exam. Sometimes the actual annual physical date is different from the information your doctor has recorded for the school.
  3. Make sure your Doctor has completed every line of the physical exam. The state has mandated all information be included. If information is missing the physical exam will be returned to your child. This will significantly delay the processing.  Check it yourself before you leave the doctor’s office.
  4. Make sure your child wears their prescription glasses to their Sports Physical appointment if they have them. If your child fails your family doctor’s vision exam, they will not be approved to play school sports. The medical thinking is uncorrected vision could lead to an avoidable sports injury.
  5. Try to get your physical done in the summer.
  6. Call your Health Insurance Carrier, know when they will pay for annual physical exams and discuss this information with your Health Care Provider.  Insurances often pay before the anniversary date.
  7. Be patient and trust that the coaches and school nurse want your child to be safe, healthy, and involved in school sports.

 

Parents Please Read: 

Our school district recognizes the importance of obtaining medical examinations. We recommend each student get at least one medical examination during each developmental stage: at early childhood (preschool through grade three), pre-adolescence (grade four through six), and adolescence (grade seven through 12).

 

 

 New Jersey Dept. of Education has required all schools to promote eye and vision safety. Our school is required to distribute this fact sheet to all students in an effort to prevent and reduce the number of sports related eye injuries. Please read and incorporate these healthy suggestions:

HOPEWELL CREST SCHOOL HEALTH NOTICE

   

Dear Parents of a Current Fifth OR Sixth Grader:

 

As of September of 2008, as per N.J.A.C. 8:57-4.24, Every child 11 years old shall receive one dose of Tdap and one dose of Meningococcal-containing vaccine. As the school nurse, I am legally responsible to review the immunization of all students enrolled at Hopewell Crest School and ensure their compliance with State Regulations.

 

             In accordance with N.J.S.A. 18A: 40-21.2, the New Jersey Department of Education has developed the attached fact sheet about Meningitis.  All public schools are required to distribute this fact sheet to parents and guardians of every student entering Grade 6.

 

             Should you have any questions about Meningitis or the Meningitis vaccine, please consult your primary care physician.

 

             Please note: Tdap Vaccine and Meningococcal Vaccine are required by the State of New Jersey upon turning 11 years old and entering Grade 6. For more information see N.J.A.C. 8:57-4 and N.J.A.C. 8:57-6. I need proof of vaccination through the New Jersey Immunization Information System (NJIIS) or an official notice from your physician.

 

 

New Jersey Department of Health has mandated any child who is 11 years old must have Tdap and MCV4 BEFORE starting grade 6.

 

Unvaccinated students must be EXCLUDED from school.

Tdap is required for students who are 11 years old before entering Grade 6. Tdap protects children from Tetanus (Lock Jaw) Diptheria and Pertussis (Whooping Cough).

 

Meningococcal Vaccine (MCV4) is required for students who are 11 years old before entering Grade 6. MCV4 & MPSV4 protects children from bacterial meningitis.

Dear Parents,

Approximately 8 years ago high school athletes and professional athletes were fainting and dying during practices.  Exact causes varied, however sports clearances came under heavy scrutiny.  School nurses worked with the Department of Education in New Jersey to reform the practice of “herd” physical exams for sports clearance. The end result is the sports physical rules we have today. Frequently asked questions and clarifications are posted on the web. All New Jersey schools are following the same rules (Laws) we must follow here at Hopewell Crest School. We have established deadlines for forms to be handed in because form review takes time and effort. Regulations have been adopted to promote good health practices and keep children safe during school sponsored activities.

Your child’s School Sports Physical is regulated by the State of New Jersey. The specifics of these regulations are found at  Basically the rules are:

  1. Have your child’s physical exam done by your family doctor. She or he should see your child well and healthy.
  2. The physical is valid for 365 days. To keep your child’s physical in good standing make your appointments for exactly 365 or as close as possible. Call the 1-800 number on your insurance card for specific Annual Physical Exam coverage. Very often your insurance plan will allow and fully cover your child’s physical, 30 days prior to the anniversary of your child’s last physical.
  3. The State of New Jersey has created the School Sports Physical Form. It is the only form acceptable.
  4. The State of New Jersey has outlined that every line of their form must be completed. A school Sports Physical is not valid and the child cannot play if information spaces are left blank.
  5. The State has mandated that the parent required Health History accompany the Physical Exam Form, be reviewed by the examining physician and remain attached to the Physical Exam Form.
  6. The State has mandated that, based on your Family Physician’s Examination, the School Physician must send a notice to each parent stating Approved or Not Approved for school sports.

Please remember all medications; Over-the-counter, and prescription medications, must be brought into school by a parent or guardian. Also a written order by the prescribing physician is required.

Why Can’t the School Nurse Give My Child Tylenol? And Other Questions You May Have Asked About the School Nurse.

 

Q: Why can’t the school Nurse give my child Tylenol?

 

A: The school nurse is a Registered Nurse (RN) legally licensed to practice nursing by the State of New Jersey. Registered Nurses by law cannot diagnose or prescribe any medication. If a school nurse (RN) gave your child Tylenol without a Medical Doctor’s (MD) order she would be diagnosing that your child has a simple headache and she would be prescribing a medication i.e. Tylenol. The school nurse cannot do this by law. To give medications in school New Jersey State Law requires signed parent/guardian permission and a complete and signed Doctor’s order. 

 

Q: Why can’t my child get a physical to play sports at school?

 

A: Starting September 2004, Sports Physicals must be done by your family physician. Your family doctor needs to see your child for a well visit to better care for them when they become ill. Having a parent present to discuss medical concerns or past medical history enables your family doctor to perform a much more comprehensive physical exam. Q: Why do I have to fill out a Health History Questionnaire every 90 days for my child to play sports?

 

A: This required by New Jersey State Law. Every student is required to have one physical per year. If the physical has been 90 days before the first day of practice or try-outs then the parent must complete and sign a Health History Update.

 

Q: Why does the school nurse bother me about my child’s immunization?

 

A: State Law requires the school nurse to insure that all students have been immunized in accordance with State Law requirements. The Cumberland County Health Department visits every school every year to make sure each school nurse is doing her job. Immunization compliance is very important with international travel so prevalent. Children are currently immunized for Measles, Mumps, Rubella (German Measles), Polio, Diphtheria, Pertussis (Whooping Cough), Tetanus, Haemophilus Influenza Type B (HIB) and Hepatitis B, Mennigitis, Varicella (Chicken Pox). These diseases are not often seen in the U.S. but they are all still present in other countries. The diseases that immunizations are protecting our children from are very serious diseases. They can lead to paralysis, brain damage, liver disease, and liver cancer and in many cases death.

 

Q: Why does the school nurse send children home with a fever?

 

A: In school everyone works closely in a closed environment. Children with fever are actively contagious to other children and adults in the school. To prevent epidemic flu and virus situations our school policy requires children with a fever to go home and remain there until at least 12 to 24 hours after a fever has lifted.

 

Q: What kind of health testing will the school nurse do on my child and how often?
