Seasonal Paperwork Requirements for Athletic Eligibility:
Regardless of grade, regardless of season, there is paperwork required each season that an athlete intends to participate. It doesn’t matter if an athlete participates in only one season per school year or three seasons in a row—the student athlete is required to submit paperwork to the nurse’s office prior to the start of EACH season.
Paperwork is due to the nurse’s office by the following deadlines:
Fall Sports due by August 1st
Winter Sports due by October 25th
Spring Sports due by February 18th*
Prior to the start of the season, the following forms are required to be on file in the health office in order to be eligible to participate in athletics:
1. Physical Evaluation by Physician: A physical is valid for 365 days; therefore, only needs to be submitted once a year. It is important to verify that all information is completed by the physician and it is stamped, signed, and dated by the office personnel. Any incomplete forms will be returned to the student athlete and may delay participation. If you need to confirm if the physical is good for the upcoming season, please call the nurse's office at 732-431-8360 ext 2038.
2. Health History Questionnaire: This form requires a parent/guardian signature. There are two types of this form, only one needs to be completed for submission each season:
¨ A long version is completed and brought to the physician’s office when receiving the annual physical.
¨ A modified (short) version should be completed when the physical on file in the health office is current. This form does not need to be reviewed by the physician.
3. Athletic Eligibility Form: This is required for all student athletes and must be submitted each season. It requires the signature of the parent/guardian as well as the student athlete.
4. Emergency Card, Concussion Acknowledgment Form and Steroid Testing form: required for all student athletes each season. It requires the signature of the parent/guardian as well as the student athlete.
