Dear Parents:

The Florida Department of Education is working with school districts to gather information on how well parents think their child's school is partnering with parents of students with disabilities, and would like to include your input. Your responses will help guide efforts to improve services and results for children and families.

This survey is for parents of students with individual educational plans (IEP) receiving special education services during the 2009-10 school year. All information provided will remain confidential. You may submit one online survey for each child receiving services.

Please visit the web site http://www.esesurvey.com/Login.php and answer this survey. This survey is designed to provide information to the state and federal government on how well schools are supporting parental involvement in their child’s education. 
Please be assure that neither parent nor their child can be identified by your answers.  Responses are confidential and names will not appear on any reports.   Survey results will be posted by FLDOE later in the fall.  Please visit the website below and complete the survey.
