The Enrollment Process at CHS

Parents must request records for review.  Crescent High School will submit an official records request form after receiving the following documentation from the parent.  The student should be in good standing with their previous school and reside with a parent/guardian in Anderson School District 3.  Click here for a printable version of this form.

_____ Social Security Card

 

_____ Immunization Records (a temporary form will be issued that is good for the first 30 days of school)

 

_____ Parent/Guardian Documentation (see explanation below)

 

_____ Proof of Residency (must be a tax notice, rent agreement, etc.)

 

_____ Withdrawal Form

 

_____ High School Transcript

 

_____ Discipline Records

 

_____ Attendance Records

 

 

 

A.)  In the event a student is not living with a natural parent/guardian, custody papers indicating the name of the person/persons given parental rights must be presented to the school before the student can enroll. 

• The Department of Social Services has the authority to place a child in a selected home.  In this event, DSS will provide the necessary documentation.
• A family court judge has the authority to place a child in a selected home.  In this event, the court will provide the necessary documentation.

 

B.)  If the parents have been divorced, the parent with whom the child is residing must present custody papers indicating which parent has legal custody. 

 

C.) If the parents have been divorced and share joint custody, the parent with whom the child is residing must present custody papers indicating placement of the child and/or educational responsibilities.
