Dear Parent/Guardian:

 

Please note the following school policies regarding the Manalapan High School Health Office:

 

 

ATTENDANCE: Please notify the Attendance Office of all absences/lateness.

 

MEDICATIONS: All medications, including over-the-counter medications, cannot be given during the school day without a doctor’s order written on the Freehold Regional High School District Medication Form; these forms are available at www.frhsd.com/manalapan. The medication must be brought to the Health Office by an adult.  Medications must be in the original, labeled pharmacy container. If your child has asthma and uses an inhaler, a doctor’s note is also required.  New medication forms are required each school year.

 

HEALTH CONCERNS: If your child has a health issue (i.e. medical problems, Epi-pen, etc.), please notify the school nurse. A “Permission to Release Confidential Information” form is available on the Health Office page on the Manalapan High School website and is recommended so that pertinent staff can be apprised of your child’s needs and health concerns. Please be assured that this information will be shared only with appropriate faculty and staff members. If you have any questions or concerns regarding the sharing of your child’s health information, please contact the health office at ext. 5046.

 

PHYSICAL EDUCATION RESTRICTIONS: If your child is unable to participate in physical education class, a doctor’s note is required with the following information:

  • Diagnosis
  • Anticipated return to physical education date. Please be advised that “until further notice” notes will only allow two weeks.
  • If applicable, the doctor’s note must indicate that your child must use crutches and/or the use of the elevator.

 

EMERGENCY CARDS:  Both sides of the emergency card must be completed and signed by the student’s parent/guardian. If emergency cards are not returned, the student will not be assigned a hall locker. Please have your child return the card to school promptly. It is important to provide at least two emergency contacts in case we are unable to reach you.

 

PHYSICALS: At least one physical exam is recommended during your child’s high school years. This form is available at www.frhsd.com/manalapan.
