Sixth Grade Health Contract

Parents: During 6th Grade Health it is the responsibility of the student to complete and hand in all assignments on time. All class requirements, due dates and assignments will be made clear to students and posted on my teacher website. We will cover multiple topics during the quarter, including but not limited to the following: character development, personal growth and wellness, social and emotional health, decision-making and goal setting, alcohol, tobacco, drugs, and nutrition. Depending on school calendar changes, lessons may be adjusted to reach the goals of each topic.
Grading will be based on the following:
1) 50 Points for Participation - Everyone starts with 50 points. Points may be deducted based on effort, attitude, and behavior. Participation points will be deducted in the amount of 1-3 points depending on the situation.
2) 10 Points for Homework Assignments - Two homework assignments will
be given at a value of 5 points each. Homework assignments will lose 1 point for each day late, and will be graded on a scale of 1-5, with 5 showing full effort and 1 showing minimal or no effort.
3) 20 Points for Quizzes - Two quizzes will be given at a value of 10 points each. Each quiz will have 20 questions.
4) 20 Points for Nutrition Project - One nutrition project will be given at a value of 20 points. The nutrition project will be an ongoing collection of breakfast and lunch data from each student's daily diet. The data will be recorded at the start of each class and will include breakfast and lunch from that day. Data will be organized and evaluated by each student at the end of the quarter. The nutrition project will be graded as follows: 17-20-Finished with excellent effort; 13-16-Finished with good effort; 9-12-Finished with partial effort; 5-8-Finished with minimal effort; 1-4-Finished with no effort.
5. A total of all four grades will be added together for each student's final score.
The first homework assignment is to have a parent sign this form and return by the next class period.

NUTRITION PROJECT:
The NUTRITION PROJECT will be an ongoing recording of nutrition data. Students will need their IPad available each day to record their breakfast and lunch meals eaten on that Health day. Data will be organized and evaluated by each student at the end of the quarter. There will be a due date established near the end of the quarter. The final project will be emailed to the teacher for grading.
